With the help of your personal trainer, you greatly improve your ability to accomplish your training goals faster, safer, and with maximum benefits. The details of these training sessions can be used for a lifetime. In order to maximize progress, it will be necessary for you to follow program guidelines during supervised and (if applicable) unsupervised training days. Remember, exercise and healthy eating are EQUALLY important! During your exercise program, every effort will be made to assure your safety. However, as with any exercise program, there are risks, including increased heart stress and the chance of musculoskeletal injuries. In volunteering for this program, you agree to assume responsibility for these risks and waive any possibility for personal damage by signing our liability waiver. You also agree that, to your knowledge, you have no limiting physical conditions or disability that would preclude an exercise program.
You accept full responsibility for your own health and well-being AND you acknowledge an understanding that no responsibility is assumed by Allen Barbell or Allen Barbell Personal Trainers.
It is recommended that all program participants work with their personal trainer three (3) times per week. However, due to scheduling conflicts and financial considerations, a combination of supervised and unsupervised workouts is possible.
Terms and Conditions for Personal Training
The billing period is four (4) weeks for any subscription plan. Subscription plan rates are based on number of sessions per week. Your chosen form of payment will be automatically charged every 4 weeks and the number of sessions in your plan will reset until you notify us to cancel.
Allen Barbell operates on a scheduled hourly appointment basis for private personal training sessions. Therefore, personal training sessions that are not rescheduled or canceled 12 hours in advance will result in forfeiture of the session and a loss of the financial investment at the rate of one session.
Sessions must be rescheduled within the same 4-week billing period.
Unused sessions do not roll over.
Should my personal trainer cancel a session with less than 12-hour prior notice, I shall be entitled to one free session upon rescheduling. I understand that the free session must be redeemed within 7 days of cancelling the previous one or shall be forfeited. However, should my personal trainer cancel a session with MORE than 12-hour notice, I shall be entitled to no free session, but the training session shall be rescheduled within 7 days to ensure consistency and progress in my fitness program.
Clients arriving late to their session will receive the remaining scheduled session time, unless other arrangements have been previously made with the trainer.
No personal training refunds will be issued for any reason, including but not limited to relocation, illness, and unused sessions.
To cancel or change your subscription plan, you agree to provide 30 days notice. Send an email to jim@allenbarbell.com 30 days before the next billing cycle to ensure automatic payments can be cancelled or revised in a timely manner.
Subscription plans will be cancelled automatically if payment is not received.
Testimonial and Photo Release
Below is a copy of the “Testimonial and Photo Release Form”. If you choose not to have your testimonial or photo used in our marketing or social media, please do not complete the form.

In consideration of good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I, the undersigned, hereby grant to James Laarveld of Allen Barbell (“Personal Trainer”) and his/her agents the right to use my name, biographical information, photographs, images, story and/or testimonial, in whole or in part, and without restriction as to changes or alterations. The rights granted herein shall extend in perpetuity, unless revoked in writing to Personal Trainer by me, throughout the world and for any purpose whatsoever, including without limitation for marketing and advertising purposes of Personal Trainer, and in any and all media,
including without limitation Personal Trainers’s website or social media. I acknowledge that Personal Trainer has no obligation to return any photographs or images to me.
I hereby RELEASE, WAIVE and FOREVER DISCHARGE any and all claims arising out of, or in connection with, such use by Personal Trainer, including without limitation any and all claims for libel or invasion or privacy.
I hereby warrant and represent that I am at least 18 years of age and have the right to contract in my own name. I have read the above Release and am fully familiar with the contents thereof.
This Release contains the entire agreement between the parties hereto as to the subject matter contained herein.

7. CONTROLS FOR DO-NOT-TRACK FEATURES
